How to provide proof of vaccination at the U of G

U of G has released an online tool to submit proof of vaccination and exemption requests

On Sept. 2, details for proof of vaccination were released by the U of G two weeks after it announced its vaccine mandate. Faculty, staff, students, and visitors are expected to be vaccinated or have approved exemption documents before entering any buildings or facilities on campus.

The U of G Proof and Exemption System can be accessed through Central Login credentials and will be used for document submission and follow-ups. The university says it is used only for verification purposes and will not store passwords.

Varsity athletes previously submitted proof of vaccination through the Department of Athletics, but are now required to resubmit proof of vaccination through the new system.

Those submitting their proof of vaccination must provide an image or PDF file of their vaccination receipts. Individuals vaccinated in Ontario may download their receipts from the Government of Ontario website while others must upload English documentation containing the date, country and location in which the vaccines were administered, the name of the administered vaccines, and if applicable, a translation of documents by a certified translator.

Students, faculty, and staff may be granted an exemption based on medical or other grounds protected under the Ontario Human Rights Code. To request an exemption, individuals must complete either a statement of creed/religious belief or a medical statement.

Students who submitted or received a vaccination exemption before Aug. 30 must now complete the new documentation and have it approved. Those with exemptions can still choose to be vaccinated at any time.

However, all those who receive an approved exemption or have only received their first dose of the vaccine must complete COVID-19 rapid testing at home no more than 72 hours before coming to campus. The testing kits are being provided by the university (through the Ontario Ministry of Health) at no cost to faculty, staff, and students who require them. Test results must be reported with photo proof through the U of G Rapid Test Submission Tool, which the university says will be available soon.

The U of G states that details on where rapid testing kits can be acquired and what will be expected from visitors are coming soon.
